MINOR PARTICIPATION & CONSENT
Minor Waiver & Parent/Guardian Consent
This waiver addresses a minor’s participation in tennis training, simulator use, coaching, drills, games, programs, camps, parties, and events at SmartServe Tennis Lab.
Legal Review Required Before Customer Use
The SSTL developer packet states that this waiver must be reviewed and approved by SSTL’s Georgia attorney and insurer before customer use. The approved final waiver should be placed in the electronic booking/account acceptance flow. This page should not be used as the final legal waiver until that approval is complete.
Parent/Guardian Acknowledgment
The parent or legal guardian acknowledges the physical risks associated with the minor’s participation in tennis training, simulator use, coaching, drills, games, programs, camps, parties, and events.
The parent or legal guardian authorizes the minor’s participation and agrees to the release and assumption-of-risk terms approved by SSTL counsel.
Safety & Staff Instructions
The parent or legal guardian agrees that the minor will follow SSTL safety rules and staff or coach instructions while participating in SSTL activities.
Emergency Information & Medical Response
The final minor waiver should address emergency contact information and emergency medical response or authorization as advised by SSTL’s legal counsel.
Any medical-treatment authorization language should be supplied or approved by counsel before the final waiver is used.
Supervision & Drop-Off
The final minor waiver should address applicable supervision and drop-off expectations for the minor.
SSTL’s separate Minor Participation & Supervision Policy applies to minors participating at the facility.
Authorized Consent
The parent or legal guardian completing the waiver must confirm that they are authorized to provide consent for the minor.
The final waiver should include confirmation of the signer’s authority to consent for the minor.
Electronic Signature
The final minor waiver should include an electronic signature and should be accepted through SSTL’s booking or account flow.
Waiver acceptance should be date- and time-stamped as part of the electronic acceptance process.
Photo & Media Consent
Optional photo and media consent should be a separate affirmative choice rather than being embedded as mandatory participation consent.
For minors, required marketing/media consent should be provided by the parent or legal guardian through the appropriate consent process.
Final Waiver Language
The release, assumption-of-risk terms, emergency-response language, medical-treatment authorization, supervision provisions, governing-law provisions, and other legally operative terms must be supplied and approved by SSTL’s legal counsel. Do not use this draft content as the final liability release without attorney approval.
